Procedure for New Clients

 

Onboarding as a New Client: what to expect

  1. As a New Client, you will receive our New Client paperwork by email from the office manager. Please complete and return the paperwork within 24 hrs of scheduling your appointment to secure the time chosen.

  2. Once our office manager reviewed the paperwork, you will receive a Patient Dashboard Welcome email from our office manager.

For initial evaluations and therapy sessions:  

  1. Verify and compete the patient profile on your Patient Dashboard (the temporary password will expire in a few days from being sent; if your password expires, please contact the office to request a new link to be sent to you)

    1. Visit Notices & Policies and sign all consents electronically.

    2. Visit Records/Medical Records and double click Pediatric Medical History to access the medical history tabs.  Please take the time to read through each tab and answer the questions that pertain to your child. 

  2. Please provide the office manager with a front/back photo of your insurance card and photo ID.  You can do this by email or fax 847-278-0458, or by sending a secure text message to 847-796-6247. 

For developmental screenings:

  1. You will receive our New Client paperwork by email from the office manager. Please complete and return within 24 hrs of scheduling your appointment to secure the time chosen.

  2. Once our office manager reviewed the paperwork, you will receive a Patient Dashboard Welcome email from our office manager.

    1. Complete the questionnaire at least one day prior to your scheduled appointment.

  3. Your Therapist will call you to introduce themselves and get more information prior to the session. However, if you have any other questions or need to reschedule, please contact our office as soon as possible (847-223-7433 or by email).

  4. If your session is in person, please use the camera on your phone with the QR code on sign to check in when you arrive and complete your daily assessment online. This will notify our office that you are here for your child's therapist to come out.

  5. If your session is telepractice, you will be sent a zoom link to log onto at the scheduled time. 

  6. Following your session, your child’s therapist will review the session with you regarding your child’s progress and carryover for home. 

  7. Providers' written documentation can be found on the patient dashboard under the visit summaries tab.
